Our Healthcare Coordinator, Heather Olson, is in constant contact with physicians from Visiting Physician Association. As a certified medical assistant, Heather oversees care plans for all residents. Our care of elderly residents consists of a certified Individual Service Plan (ISP) and a health statement from the patient’s own physician.

The role of the care professional is more than just the implementation of the care plan. A large part of her role involves documentation, collaboration with other healthcare staff, and communicating with the client/family members about changes in the care plan or ISP.

Dear Melinda,

I wanted to personally thank you again for the care you gave my mother June. I have heard it said that there are angels among us, now I know that there is. I don’t know what we would have done without you and Personally Yours home. I know it wasn’t very easy for you, but you and your family took care of her despite all the problems. Thank you again.
