Personally Yours Elder Care, on Gunderson Road outside of Waterford, Wisconsin is one building and contains three separate houses with separate staff and rooms.

Dolphin House Units A, B

Dolphin House is a duplex located in the Town of Rochester, Wisconsin (although it has a Burlington, Wisconsin address). This pleasant, newer building contains two separate houses. Again, it is staffed at a ratio of one caregiver to four residents.

Personally Yours Eldercare is made up of five different entities within two buildings. Our Personally Yours Eldercare building is a triplex meaning there are three homes in one building. Dolphin House is a duplex meaning there are two homes within one building. Although they are all unique in physical appearance each one is operated identically and regulated by the State of Wisconsin. All homes are state licensed Adult Family Homes that accommodate up to four residents in a home setting.

Our homes are staffed 24/7 with caregivers that are educated in the types of individuals we are licensed to provide care for. If a resident requires additional care above and beyond the Service Providers, we work with outside services to meet the needs of each individual person to enhance their quality of life. Each home has adaptive equipment to aid in safely moving residents to avoid injury. Our Healthcare Coordinator is in constant contact with the physician from Visiting Physician Association (who oversees most of our residents) to monitor the care and any changes in condition of the residents.

Our Service Providers encourage each resident to maintain the ability to perform their own activities of daily living. Part of activities of daily living are promoting social interaction with residents. Our Activity Coordinator visits each house on a routine basis to engage residents in activities from exercise to playing games.

Our meals are all home cooked daily and tailored to the Physician prescribed diet for each resident. As people age, they tend to lose their sense of thirst, therefore we push fluids all day as this also helps digest food and absorb nutrients.

Family is one of the most important elements in the care that we provide to our residents. We encourage your input in our process to help drive the personalized care plan and maintain a higher standard of care.
